We MUST have the following information to send replacement card(s):

1. Your PRINTED name as it will appear on the card.
2. The date of the course you attended.
3. The location of the class.
4. Enclose a self-addressed and stamped envelope.

Due to the volume of courses we conduct, it is impossible to process a replacement card request without all of this information.

Fees for replacement cards:
CPR/AED course completion card only; $5.00 each
First Aid course completion card only; $5.00 each

CPR/AED/First Aid course completion card set $10.00

CPR/AED ASHI course completion card; $7.00

1st Aid ASHHI course completion card; $7.00

SIDS/SBS $7.00
Healthcare Provider $7.00
Water Safety $12.50

We are unable to process incomplete requests.

Upon receipt of your request, please allow two weeks for us to research the file and confirm your attendance. Thanks!
